What happens in the body of a horse with EMS? 🩵
Equine Metabolic Syndrome (EMS) is a hormonally driven condition rooted in how the horse’s body handles sugar and diet plays a central role in both its development and management 🌱
Here’s what’s happening under the surface: when a horse consumes sugar or starch, whether from grass, cereals, or molassed feeds, glucose enters the bloodstream. As a result the pancreas releases insulin, the hormone responsible for moving that glucose into muscle and fat cells. In a normal horse, this process brings blood sugar back to normal efficiently.
But in horses with EMS, the cells no longer respond properly to insulin. This is known as insulin resistance. The body compensates by producing more insulin in an attempt to lower blood glucose. Over time, this leads to chronic hyperinsulinaemia (consistently elevated insulin levels), even when glucose levels are only mildly raised.
This high circulating insulin becomes dangerous. Studies show that elevated insulin directly disrupts blood flow to the laminae in the hoof and activates cellular changes that weaken the structural integrity of the hoof capsule. This is why EMS is so strongly associated with laminitis even in horses that appear well on the outside.
